About the Program - This page gives general information and describes temporary disability benefits provided by a Private Plan carrier approved by the Sate of New Jersey.

Work Site Visit - This page provides information for having a representative from Private Plan Claims Review Unit visit your work site.  It gives the address, telephone number and fax number for the Claims Review Unit.  A link is also provided to e-mail a request to the Unit.

PLAN INFORMATION

Plan Approval - This page explains the approval process for establishing a private plan for Temporary Disability Insurance coverage.  Links are provided for a list of approved insurance companies, information for new insurers and the Self Insured Packet.  A link is provided to request a Private Plan application via e-mail.

Plan Modification - This page contains instructions for how to modify an approved Private Plan and the reason a plan can/must be modified.  It gives the address and telephone number for the Private Plan Approval Unit.
